download the membership form in Adobe PDF format. Print
the form, fill it out, and send it to us with your conference
registration or contest entry. If you don’t already have Adobe
Reader, click here to download it free.

If you are sending your membership form with your
conference registration, please be certain to enter your
payment amount on the conference registration form in the
correct box (Non-member joining PPW), and there will be no
need to send separate payment for your membership.

If you are sending your membership form with your contest
entry, then please provide separate payment for your
membership and simply check “member” on the contest entry
form and pay the member entry fees instead of those for non-
members.
